This service is for people aged 60 and over and resident in the London Borough of Enfield who are ready to be discharged home from North Middlesex hospital wards or Accident & Emergency Department. Alpha Care Specialists Take Home and Settle Workers will escort you home in their own vehicles. The service is funded by the NHS Enfield CCG and is supported by the London Borough of Enfield. During the service our worker will make sure you get home safely and can: offer you a daily call for up to 3 days following discharge, refer you on to other support services as appropriate, ensure you have sufficient food and that your home is adequately heated; prepare a light meal and undertake light housework.
This service is available on Saturdays and Sundays from 12 noon - 10pm.
