This service aims to support people following a period of illness, disability or loss of confidence. The service also helps people to remain living safely in their own homes. The service is delivered in a way that promotes people's independence, choice and quality of life; it also helps them to regain skills and confidence. The service facilitates a smooth and quick hospital discharge, promotes wellbeing and enablement and ease transition from hospital.
